If you issue a P.O.   1. Calculate your Costs - Download Software License Agreement Form (a.k.a. the “order form”) to use as a guide to stay in budget and calculate your costs. Please note: Each customer MUST select at least one Product Training & Support Option. 2. Issue the P.O. to Us – Help your accounting department by giving them our name, address, and phone number: Learning Today, Inc. 100 NW 70th Ave. Ste. 202 Plantation , Fl 33317 888-587-5250 3. Send the P.O. to Us - You can mail or fax it! Faxing will expedite your implementation. a) Mail : Add “attn: Accounting” to the address above b) Fax : 954-584-5273 attn: Accounting 4. Help Us Help You! - Download and complete the “Let’s Get Started Form” and send it in with your P.O. 5. We will Contact You! – Once we receive your P.O. and the “Let’s Get Started Form,” we will call the person you designated on this form in order to get the implementation and training processes started. 6. Complete and Sign the Order Form – Once you have made your choices on the form, complete the bottom portion and have an authorized person sign it. “In School” and “Extended-day” agreements are valid for 12 months.   Intervention agreements are valid for 14 weeks. 3. Send the signed Order Form to us - You can mail or fax it! Faxing will expedite your implementation.
